What is a dust mite allergy?
Dust mite allergy is an immune-mediated reaction to proteins produced by dust mites — microscopic arachnids in the genus Dermatophagoides that live in household dust and feed primarily on shed human skin cells. Dust mites are one of the most prevalent indoor allergens worldwide and among the most common triggers of perennial — year-round — allergic rhinitis and asthma. Unlike seasonal pollen allergies that follow a predictable calendar, dust mite allergy produces symptoms throughout the year because dust mites are present in the home environment in every season.
Two species are most clinically significant in the United States: Dermatophagoides pteronyssinus and Dermatophagoides farinae. Both species thrive in warm, humid conditions and are found in virtually every home, regardless of cleanliness. Dust mites do not bite and are not parasitic — they live entirely within the fabric of the home environment, concentrated most heavily in bedding, mattresses, pillows, upholstered furniture, carpets, and soft toys. The allergens they produce are not the mites themselves but proteins found in their fecal particles and body fragments, which become airborne when disturbed and are inhaled by household occupants.
The primary allergens in dust mite allergy include Der p1 and Der p2 (D. pteronyssinus) and Der f1 and Der f2 (D. farinae). Der p1 and Der f1 are cysteine proteases — enzymes that are capable of disrupting the epithelial barrier of the respiratory tract, enhancing allergen penetration and promoting sensitization. This mechanism partly explains why dust mite sensitization is so common and why it is strongly associated with the development of asthma as well as rhinitis.

Diagnosis
Clinical history
A detailed history is the starting point for dust mite allergy diagnosis. Your allergist will assess the pattern and timing of your symptoms, whether they are year-round or have any seasonal variation, whether they are worse in the morning or at night, whether specific environments such as the bedroom or older upholstered furniture appear to worsen symptoms, and whether time away from home — on holiday or travel — produces improvement. A history of asthma, eczema, or other atopic conditions in the patient or their family increases the pre-test likelihood of dust mite sensitization.
Skin prick test
A skin prick test is the most common first-line diagnostic tool for dust mite allergy. Standardized extracts of D. pteronyssinus and D. farinae are applied to the forearm or back and the skin is lightly pricked. A raised, itchy wheal at the test site within 15 to 20 minutes indicates sensitization. Testing for both species simultaneously is standard practice given that co-sensitization is common.
Blood test
A blood test measuring specific IgE antibodies to dust mite allergens — including component testing for Der p1, Der p2, Der f1, and Der f2 — can confirm sensitization and provide additional detail about the degree of reactivity. Blood testing is a useful alternative when skin testing is not appropriate, such as in individuals with extensive eczema or those taking medications that interfere with skin test results. Component-resolved testing can also help distinguish primary dust mite sensitization from cross-reactivity with related allergens such as shellfish, as dust mite and shellfish share a cross-reactive protein called tropomyosin.
Dust mite allergen avoidance
Environmental control measures to reduce dust mite allergen exposure are a cornerstone of dust mite allergy management. While complete elimination of dust mites from the home environment is not achievable, a combination of targeted measures can significantly reduce allergen load and improve symptom control.
Bedding
The bedroom is the highest-priority area for dust mite control because it is where most people spend the greatest proportion of their time indoors, and where prolonged close contact with allergen-laden bedding, mattresses, and pillows occurs.
- Encase mattresses, box springs, and pillows in allergen-impermeable covers with zip closures
- Wash all bedding — sheets, pillowcases, and blankets — weekly in hot water at a minimum of 60°C (140°F), which kills dust mites and removes allergen
- Replace feather or down pillows and duvets with synthetic alternatives, or encase them in allergen-proof covers
- Wash soft toys regularly or place them in the freezer for 24 hours to kill dust mites, then wash to remove allergen
Flooring and furnishings
- Remove wall-to-wall carpeting where possible and replace with hard flooring such as wood, tile, or laminate, particularly in the bedroom
- Where carpet removal is not possible, vacuum regularly using a vacuum cleaner fitted with a HEPA filter
- Replace upholstered furniture with leather, vinyl, or wood alternatives where feasible
- Minimize soft furnishings including cushions, fabric curtains, and decorative textiles in the bedroom
Humidity control
Dust mites require humidity above 50 percent to survive and reproduce. Reducing indoor relative humidity to below 50 percent significantly inhibits dust mite populations.
- Use a dehumidifier in humid rooms, particularly basements and bedrooms
- Ensure adequate ventilation, particularly in bathrooms and kitchens
- Avoid drying laundry indoors where possible as this raises indoor humidity
Air filtration
- Use HEPA air purifiers in the bedroom and main living areas to reduce airborne dust mite allergen particles
- Replace HVAC filters regularly with high-efficiency particulate filters
- Avoid forced-air heating systems where possible, or cover vents with filters to reduce allergen circulation
Treatment
Antihistamines
Second-generation H1 antihistamines are used to manage mild to moderate dust mite allergy symptoms including sneezing, itching, runny nose, and eye symptoms. Because dust mite exposure is continuous, consistent daily use is generally more effective than taking antihistamines on an as-needed basis.
Nasal corticosteroids
Intranasal corticosteroid sprays are the most effective daily medication for dust mite-induced allergic rhinitis. Used consistently, they reduce nasal inflammation and significantly improve congestion, runny nose, post-nasal drip, and sleep quality. They are the first-line pharmacological treatment for persistent allergic rhinitis caused by dust mite allergy and are most effective when used regularly rather than intermittently.
Eye drops
Antihistamine or mast cell stabilizing eye drops can provide targeted relief for ocular symptoms including itching, redness, and watering.
Asthma management
For individuals with dust mite-triggered asthma, inhaled corticosteroids and bronchodilators form the basis of medical management alongside environmental control measures. Poorly controlled dust mite allergy is a frequent driver of inadequate asthma control, and addressing the underlying allergen sensitization through immunotherapy can significantly improve asthma outcomes.
Allergen immunotherapy
Allergy immunotherapy is the only treatment that addresses the underlying immune response to dust mite allergens rather than simply managing symptoms. Dust mite immunotherapy is one of the best-studied forms of allergen immunotherapy and has strong evidence supporting its efficacy for both allergic rhinitis and asthma.
Immunotherapy can be delivered as allergy shots (subcutaneous immunotherapy) or as sublingual drops or tablets. Through a program of gradually increasing allergen exposure, the immune system is desensitized to dust mite proteins over time, producing reductions in symptom severity, medication need, and asthma exacerbation risk that can persist for years after treatment is completed.

Covers should encase the entire mattress and pillow and have zip closures — open-ended or loosely fitting covers are much less effective.
Does vacuuming help with dust mite allergy?
Regular vacuuming with a HEPA-filter vacuum cleaner can reduce surface dust and allergen levels, but vacuuming also temporarily disturbs settled dust and increases airborne allergen levels during and immediately after cleaning. Dust mite-allergic individuals should ideally leave the room during vacuuming and for 20 to 30 minutes afterward, or wear a face mask while cleaning. Vacuuming alone is not sufficient to control dust mite allergen in carpeted environments — mattress encasements and hot washing of bedding are higher-priority measures.
Is dust mite allergy related to shellfish allergy?
