What is a grape allergy?

Grape allergy is an immune-mediated reaction triggered by proteins found in grapes (Vitis vinifera and related species) and grape-derived products including wine, raisins, grape juice, vinegar, and a wide range of food and beverage products where grape derivatives appear as ingredients. While grape allergy is not among the most prevalent food allergies, it is a clinically recognized condition whose complexity is frequently underestimated — in part because grapes are consumed in so many forms, and in part because grape products such as wine contain multiple potential triggers beyond grape proteins alone.

Grapes belong to the Vitaceae family, a relatively small botanical family with limited cross-reactivity to other common food allergens. The most clinically significant allergenic proteins in grapes include lipid transfer proteins (LTPs), chitinases, thaumatin-like proteins, and a range of pathogenesis-related (PR) proteins. LTPs are particularly important in grape allergy because they are heat-stable and resistant to digestion, meaning they retain their allergenic properties in cooked, dried, fermented, and processed grape products. LTP-mediated grape allergy is associated with a higher risk of systemic reactions and is the protein class most consistently implicated in severe reactions to wine and grape products.

Grape allergy has a notable geographic distribution pattern in the published literature, with higher reported prevalence in Mediterranean countries where grape consumption is highest and where LTP sensitization is more prevalent generally. In the United States, grape allergy is less commonly diagnosed but is encountered in allergy practice, and the growing consumption of wine, grape-based supplements, and grape seed extract products has expanded the potential exposure landscape.

Wine reactions deserve particular clinical attention because wine contains multiple potential triggers beyond grape proteins. Sulfites, which are used as preservatives in most wines, are a well-documented cause of reactions in sensitive individuals. Histamine and other biogenic amines naturally present in red wine can cause flushing, headache, and nasal symptoms in individuals with histamine intolerance. Mold proteins from the fermentation process, fining agents including egg whites and casein used in winemaking, and yeast proteins can all independently cause reactions. Distinguishing true grape allergy from reactions to these other wine components requires careful clinical evaluation and targeted testing.

Grape seed extract — used widely as a nutritional supplement for its antioxidant properties — is a concentrated source of grape proteins and has been associated with allergic reactions in grape-sensitized individuals. Its increasing prevalence in health food products and supplements makes it an important hidden source of grape allergen to be aware of.

Symptoms

Grape allergy symptoms typically develop within minutes to a couple of hours of consuming grapes or grape-derived products and can range from mild to severe.

In LTP-mediated grape allergy:

  • Itching or tingling in the mouth, lips, or throat
  • Swelling of the lips, tongue, or face (angioedema)
  • Hives
  • Skin flushing or rash
  • Stomach cramps
  • Nausea or vomiting
  • Wheezing or difficulty breathing
  • Dizziness or lightheadedness
  • Anaphylaxis in severe cases
  • Both fresh and processed grape products can trigger reactions

In pollen cross-reactive grape allergy:

  • Itching or tingling in the mouth, lips, or throat
  • Mild swelling of the lips or tongue
  • Symptoms typically confined to the mouth and throat
  • Usually resolves quickly
  • Fresh grapes most commonly implicated

Symptoms that may indicate wine-specific triggers rather than grape allergy:

  • Flushing, headache, and nasal congestion after red wine specifically — more consistent with histamine intolerance or sulfite sensitivity
  • Reactions to wine but not to fresh grapes or raisins — may indicate a wine additive rather than grape protein allergy
  • Reactions reproducible with sulfite-containing products other than wine

Diagnosis

Clinical history

A detailed clinical history is the starting point for grape allergy diagnosis. Your allergist will assess which grape products have caused reactions, whether fresh grapes and processed grape products both trigger symptoms or only specific forms, whether wine reactions occur with all wines or specific types, and whether there is a background of pollen sensitization or LTP-related plant food reactivity. Differentiating grape protein allergy from sulfite sensitivity, histamine intolerance, or reactions to winemaking additives is a key early step that requires a careful review of the full reaction history.

Skin prick test

A skin prick test can assess sensitization to grape proteins. Fresh grape extract — prepared from the skin, flesh, and seed as relevant to the clinical history — is typically used alongside commercial extracts where available. Given the LTP cross-reactivity pattern, simultaneous testing for peach LTP and other LTP-containing foods may be informative.

Blood test

A blood test measuring specific IgE antibodies to grape proteins — and to grape LTP specifically — can confirm sensitization and provide information about likely reaction severity. Component-resolved testing for LTP sensitization is particularly useful in grape allergy as it identifies individuals at higher risk of systemic reactions and helps distinguish primary grape allergy from pollen-driven cross-reactivity.

Sulfite sensitivity assessment

Where wine reactions are suspected to involve sulfite sensitivity rather than or in addition to grape allergy, a clinical assessment for sulfite reactivity may be conducted. This may include reviewing reactions to other sulfite-containing foods such as dried fruits, processed meats, and pickled products alongside wine.

Oral food challenge

Where the diagnosis is uncertain or where specific grape products need to be formally assessed for safety, an oral food challenge may be recommended under direct medical supervision.

Treatment

Avoidance

The extent of avoidance depends on the allergenic protein responsible and individual clinical history. For LTP-mediated grape allergy, avoidance of grapes and grape-derived products in all forms is required. For pollen-driven reactions with mild oral symptoms, fresh grape avoidance may be sufficient while processed products are tolerated.

Grape is not a named major allergen under US food labeling law and will not always be prominently disclosed on labels. Careful ingredient review is essential, particularly for:

  • Wine and wine-based drinks including mulled wine and sangria
  • Raisins, sultanas, and currants in baked goods, cereals, and trail mixes
  • Balsamic vinegar and wine vinegar in dressings, marinades, and sauces
  • Grape juice in fruit juice blends
  • Cream of tartar in baked goods and stabilized whipped cream
  • Grape seed extract in nutritional supplements and antioxidant products
  • Grape seed oil in cooking oils and salad dressings
  • Cosmetic and skincare products containing grape seed extract, resveratrol, or grape-derived ingredients

Antihistamines

Antihistamines can be given after epinephrine administration to help block the histamine-mediated portion of an allergic reaction. They should not be depended on as a standalone treatment for food allergies.

Epinephrine

Individuals with a history of severe grape reactions or confirmed LTP sensitization should carry an epinephrine auto-injector (such as an EpiPen, Auvi-Q, or neffy intranasal epinephrine) at all times. Epinephrine is the first-line emergency treatment for anaphylaxis and should be used at the first sign of a severe systemic reaction, followed immediately by calling emergency services.

Why do I react to red wine but not white wine?

Reactions specific to red wine — particularly flushing, headache, and nasal congestion — are more consistent with histamine intolerance or tannin sensitivity than with true grape protein allergy. Sulfite content also varies between wines, though white wines often contain equal or higher sulfite levels than red wines. A detailed reaction history and targeted testing can help identify which component is responsible.

Are raisins as allergenic as fresh grapes?

For LTP-mediated grape allergy, yes. LTPs are heat-stable and concentrate during the drying process, meaning raisins, sultanas, and currants can be equally or more reactive than fresh grapes in LTP-sensitized individuals. For pollen-driven allergy involving heat-labile proteins, the drying process may reduce reactivity, though this should not be assumed without allergist guidance.

Is grape seed extract in supplements safe if I have a grape allergy?
