Is wine allergy real?

Reactions to wine are common — studies suggest that up to a third of people report some form of adverse reaction to wine at some point. However, the term “wine allergy” covers a range of distinct conditions with different underlying mechanisms, and understanding which one is responsible for your symptoms is the key to managing them effectively.

True IgE-mediated allergy to wine — meaning an immune response to grape proteins specifically — is relatively uncommon. The majority of wine reactions involve one of several other mechanisms: sensitivity to sulfites used as preservatives, intolerance to histamine and other biogenic amines naturally present in wine, reactions to proteins from the fermentation process including yeast and mold, or reactions to fining agents such as egg whites, casein, or isinglass used during winemaking. Some individuals have reactions driven by more than one mechanism simultaneously.

This complexity means that self-diagnosing a wine reaction — and particularly self-managing it through trial and error with different wine types — is unreliable and can lead to ongoing exposure to the actual trigger. A formal assessment by an allergist is the most reliable way to identify what is causing your reaction and how to manage it.

The main causes of wine reactions

Grape protein allergy

True allergy to grape proteins — particularly lipid transfer proteins (LTPs) — produces consistent reactions to fresh grapes, raisins, grape juice, and wine alike. LTPs are heat-stable and retained through the fermentation and aging process, meaning wine made from cross-reactive grapes can trigger reactions in LTP-sensitized individuals. Reactions tend to be systemic — hives, angioedema, breathing difficulty — rather than confined to flushing or nasal symptoms.

Sulfite sensitivity

Sulfites — including sulfur dioxide, potassium metabisulfite, and sodium bisulfite — are used as preservatives in virtually all commercially produced wines to prevent oxidation and microbial spoilage. In the United States, wines containing more than 10 parts per million of sulfites must carry a “contains sulfites” label. Sulfite sensitivity is not a true IgE-mediated allergy but a pharmacological hypersensitivity that can trigger asthma, rhinitis, hives, and in rare cases anaphylactoid reactions in sensitive individuals.

A common misconception is that red wine contains more sulfites than white wine. In fact, white wine and sweet wines often contain equal or higher sulfite concentrations than red wines because sulfites are needed to prevent re-fermentation in sweeter styles. Red wine reactions are more commonly driven by histamine and tannins than by sulfites.

Histamine intolerance

Red wine contains significantly higher concentrations of histamine than white wine, produced during the malolactic fermentation process that most red wines undergo. Histamine intolerance occurs when the body’s capacity to break down histamine — primarily through the enzyme diamine oxidase (DAO) — is insufficient to process the histamine load from consumed foods and beverages. Symptoms include flushing, headache, nasal congestion, and skin redness, and are characteristically more pronounced with red wine than white. Histamine intolerance is not an allergic condition and does not involve IgE antibodies.

Biogenic amines

Beyond histamine, wine contains other biogenic amines including tyramine, putrescine, and cadaverine that can trigger reactions in sensitive individuals. Tyramine in particular is associated with migraine headaches following red wine consumption in susceptible individuals.

Yeast and mold proteins

Wine fermentation involves yeast — most commonly Saccharomyces cerevisiae — and some wines are produced or aged in conditions that involve mold exposure. Individuals with yeast or mold allergy may react to residual yeast proteins in wine, particularly in unfiltered or naturally fermented wines. This is a less common cause of wine reactions but is worth considering in individuals with known mold allergy who react specifically to certain wine styles.

Fining agents

Winemakers use a range of fining agents to clarify wine and remove unwanted proteins, tannins, and other compounds before bottling. These include egg whites (albumin), milk protein (casein), fish-derived isinglass, and gelatin. While most fining agents are removed from the final product, trace amounts can remain and potentially trigger reactions in highly sensitive individuals with egg allergy, milk allergy, or fish allergy. EU labeling law requires disclosure of egg and milk-based fining agents on wine labels — US labeling requirements are less stringent.

Tannins

Tannins are polyphenolic compounds present in grape skins, seeds, and stems and in oak barrels used for aging. Red wines contain significantly higher tannin levels than white wines. Some individuals report headaches, nasal congestion, and skin reactions after red wine that may be tannin-related, though the evidence base for tannin sensitivity as a distinct clinical entity is less robust than for sulfites or histamine.

Symptoms

Wine reaction symptoms vary considerably depending on the underlying mechanism and can overlap significantly between the different causes.

Symptoms suggesting true grape allergy (IgE-mediated):

  • Consistent reactions to both wine and fresh grapes, raisins, or grape juice
  • Hives
  • Swelling of the lips, tongue, or face (angioedema)
  • Throat tightening or wheezing
  • Dizziness or lightheadedness
  • Anaphylaxis in severe cases
  • Reactions occur with all wine types

Symptoms suggesting sulfite sensitivity:

  • Worsening of asthma symptoms — wheezing or chest tightness — shortly after drinking wine
  • Reactions also occur with other sulfite-containing foods such as dried fruit, processed meats, and pickled products
  • Reactions not confined to red wine specifically

Symptoms suggesting histamine intolerance:

  • Flushing and skin redness
  • Headache or migraine
  • Nasal congestion and runny nose
  • Symptoms more pronounced with red wine than white
  • Similar symptoms after other high-histamine foods including aged cheeses, cured meats, and fermented foods

Symptoms suggesting tannin sensitivity:

  • Headache following red wine specifically
  • Nasal congestion
  • Symptoms also occur after strong tea or dark chocolate

Symptoms suggesting fining agent reaction:

  • Reactions in individuals with confirmed egg, milk, or fish allergy
  • Reactions variable between wines of different producers

Diagnosis

Identifying the specific cause of a wine reaction requires careful clinical assessment. Because the mechanisms differ considerably, the diagnostic approach varies depending on which cause is suspected from the clinical history.

Clinical history

A detailed history is the most important diagnostic tool. Your allergist will assess which types of wine trigger reactions, whether fresh grapes or other grape products also cause symptoms, whether reactions occur with other sulfite-containing or high-histamine foods, the exact nature and timing of symptoms, and whether there is a background of asthma, atopic conditions, or known food allergies that might predispose certain reaction types.

Key questions that help distinguish the cause include: Do you react to all wines or specific types? Do fresh grapes or raisins cause the same reaction? Do dried fruits, processed meats, or pickled foods also cause symptoms? Are symptoms worse with red wine specifically? Do you also get headaches from aged cheese or cured meats?

Skin prick test and blood test

A skin prick test and specific IgE blood test for grape proteins — including grape LTP — can assess for true IgE-mediated grape allergy. These tests are not useful for diagnosing sulfite sensitivity, histamine intolerance, or fining agent reactions, which are not IgE-mediated.

Sulfite sensitivity assessment

Where sulfite sensitivity is suspected, a clinical assessment of reactions to other sulfite-containing foods and beverages provides useful supporting evidence. A supervised sulfite challenge may be considered in specialist settings.

Patch testing

Patch testing may be relevant in individuals who develop contact skin reactions from handling wine or wine-soaked materials.

Treatment and management

For true grape protein allergy

Strict avoidance of wine and all grape-derived products is required.

For sulfite sensitivity

Avoidance of high-sulfite wines and other sulfite-containing foods is the primary management approach. Individuals with asthma and sulfite sensitivity should ensure their asthma is well controlled with appropriate inhaled medications, as sulfite-triggered asthma can be severe. Organic wines are sometimes marketed as sulfite-free but in reality contain naturally occurring sulfites from fermentation — they simply have no added sulfites, which may reduce but does not eliminate sulfite content.

For histamine intolerance

Choosing lower-histamine wine styles — white wines, rosé, and wines that have not undergone malolactic fermentation — can reduce symptom burden. Diamine oxidase (DAO) enzyme supplements taken before consuming high-histamine foods or wine have been used by some individuals to improve histamine tolerance, though the evidence base is limited. Antihistamines taken before wine consumption may also reduce symptoms, though this approach masks the reaction rather than addressing the underlying intolerance.

For fining agent reactions

Choosing vegan wines — which use plant-based fining agents rather than egg, milk, or fish-derived products — is the most practical management approach for individuals reacting to fining agents. Vegan wine certification is increasingly available and many producers label their wines accordingly.

Antihistamines

Antihistamines can be given after epinephrine administration to help block the histamine-mediated portion of an allergic reaction in true grape allergy. They should not be depended on as a standalone treatment for food allergies. For histamine intolerance specifically, antihistamines may reduce symptom severity but do not address the underlying intolerance mechanism.

Epinephrine

Individuals with confirmed true grape allergy and a history of severe or systemic reactions to wine should carry an epinephrine auto-injector (such as an EpiPen, Auvi-Q, or neffy intranasal epinephrine) at all times. Epinephrine is the first-line emergency treatment for anaphylaxis and should be used at the first sign of a severe systemic reaction, followed immediately by calling emergency services.

Frequently asked questions

Why do I react to red wine but not white wine?

Reactions specific to red wine are most commonly driven by histamine or tannins rather than grape protein allergy or sulfite sensitivity. Red wine contains significantly higher histamine concentrations than white wine due to malolactic fermentation, and much higher tannin levels due to extended skin contact during production. Flushing, headache, and nasal congestion after red wine specifically — while white wine is tolerated — points strongly toward histamine intolerance or tannin sensitivity rather than true grape allergy.

Does organic wine have less sulfites?

Organic wine contains no added sulfites but still contains naturally occurring sulfites produced as a byproduct of fermentation. Sulfite levels in organic wine are lower than in conventionally produced wine but are not zero. For individuals with asthma triggered by sulfites, organic wine may be better tolerated, but it cannot be assumed sulfite-free. Natural and biodynamic wines similarly vary in sulfite content.

Can I test whether I am reacting to sulfites by eating dried fruit?

Dried fruits — particularly dried apricots, raisins, and prunes — are among the highest-sulfite foods in the diet and can be a useful informal indicator of sulfite sensitivity. If you consistently react to dried fruit with similar symptoms to your wine reactions, sulfite sensitivity is a plausible explanation. However, dried fruit also contains other compounds that can cause reactions, and formal clinical assessment is more reliable than self-testing with foods.

Could my wine reaction be caused by the alcohol itself?

Alcohol itself can cause facial flushing, particularly in individuals of East Asian descent who have reduced activity of the enzyme aldehyde dehydrogenase — a condition sometimes called alcohol flush reaction. This is a metabolic enzyme deficiency rather than an allergy and produces flushing and nausea with all alcoholic beverages rather than wine specifically. If you react to all forms of alcohol equally, alcohol metabolism rather than a wine-specific ingredient is the more likely explanation.

Are there wines less likely to cause reactions?

For histamine intolerance, white wines — particularly those that have not undergone malolactic fermentation, such as most Rieslings, Pinot Grigios, and Sauvignon Blancs — tend to have lower histamine levels than red wines. For sulfite sensitivity, lower-alcohol wines and wines with lower residual sugar tend to require less sulfite addition. For fining agent reactions, vegan-certified wines avoid animal-derived fining agents. For true grape protein allergy, no wine style is safe and avoidance of all wine and grape products is required.

Is wine allergy the same as alcohol intolerance?

No. Alcohol intolerance — most commonly the alcohol flush reaction seen in individuals with aldehyde dehydrogenase deficiency — affects all alcoholic beverages equally and reflects a metabolic enzyme deficiency rather than an immune or hypersensitivity reaction to wine-specific components. Wine reactions caused by grape proteins, sulfites, histamine, or fining agents are specific to wine or to ingredients shared across certain food categories, rather than to alcohol per se. An allergist can help determine which category your reactions fall into.
